Step 1:

PRE-REGISTER

Be on the lookout for an email or text message welcoming you to our new surgical patient portal. This portal will allow you to complete your pre-surgical questionnaire online instead of answering these questions over the phone.

DO I NEED TO DOWNLOAD AN APP FOR THIS?

No, by clicking the link in the email or text message, you will be brought to the login screen in your web browser.

DO I NEED TO CREATE AN ACCOUNT AND PASSWORD?

No, your secure link will allow you to log in using your name and date of birth.

WHAT IF I HAVE ALREADY COMPLETED PRE-REGISTRATION FOR A PREVIOUS PROCEDURE?

You do not need to complete the questionnaire again, but one of our nurses will contact you to ask for any updates to your health history.

Please make sure to complete pre-registration at least 48 hours prior to your procedure and contact us if you have any additional questions.

Step 2:

PREPARE

Here’s important information to make your experience the best possible:

A NURSE WILL CALL YOU the day before your procedure to confirm your procedure time.

FOLLOW ALL THE PRE-OP INSTRUCTIONS given to you by your physician and/or our Center. If you have any questions about this, please call your surgeon’s office directly.

YOU MUST HAVE AN ESCORT to drive you home after your procedure and to stay with you for 24 hours following your surgery.

PLEASE BRING THE FOLLOWING to your surgical appointment:

– A form of identification (i.e., driver’s license)

– Insurance card

– Any expected co-pays or deductibles

– Crutches, sling, walker, cane, etc.

REVIEW THE PRE-ADMISSION DOCUMENTS located below on our website prior to your procedure. They contain legally required information for your review and acknowledgment.

PLEASE DO NOT WEAR ANY JEWELRY, CONTACT LENSES OR NAIL POLISH and do not bring any valuables with you. You are valuable enough!
